South Dakota: Brain Injury Symptoms
The Rancho Los Amigos scale measures the ability of cognitive functioning in a person who has experienced a traumatic brain injury. Many South Dakota brain injury victims have their cognitive levels measured by this method. There are a total of ten levels in the scale as follows.
Level One: No response, total assistance
- No change in behavior after exposure to various stimuli
Level Two: Generalized response, total assistance
- Shows generalized reflex response to painful stimuli
- Responds to repeated auditory stimuli
- Responds to external stimuli with physiological changes
- Responses are significantly delayed
Level Three: Localized response, total assistance
- Shows withdrawal or vocalization to painful stimuli
- Turns away or to auditory stimuli
- Blinks when presented with a light
- Responds to discomfort
- Inconsistently responds to basic commands
- May respond to family or friends
Level Four: Confused or agitated, maximal assistance
- Attempts to remove restraints or tubes or leave the bed
- May sit, reach or walk without purpose
- No short term memory
- May become aggressive or experience mood swings
- Incoherent or inappropriate words
Level Five: Confused inappropriate, not agitated, maximal assistance
- Alert and not agitated
- Not oriented to people, places or times
- Brief periods of sustained attention
- Confusion of past and present
- Unable to learn new tasks
- Able to converse for brief periods, upon commands
Level Six: Confused, appropriate, moderate assistance
- Vague recognition of hospital staff
- Consistently follows basic directions
- Verbalizations are appropriate
- Supervised, but carries over some information about simple tasks
- Unaware of impairments or difficulties
Level Seven: Automatic, appropriate, minimal assistance
- Consistently oriented to people, place and time
- Overestimates abilities
- Little supervision for new learning
- Uncooperative
- Carries over information about basic skills
Level Eight: Purposeful, appropriate, stand-by assistance
- Depressed, self-centered and irritable
- Independently completes tasks
- Able to recall past and recent events
- Requires no assistance on newly learned tasks
- Able to recognize inappropriate behavior
Level Nine: Purposeful, appropriate, stand-by assistance on request
- Can recall daily schedule
- Aware of impairments or disabilities
- Aware of other’s feelings
- Able to think about consequences
- Able to handle multiple tasks
- Social interaction is consistently appropriate
- Anticipates impact of disabilities on daily activities
- Independently carries out household tasks
Level Ten: Purposeful, appropriate, modified independent
